Bupivacaine hydrochloride is a long-acting local anesthetic of the amide type, its anesthesia time is 2 to 3 times longer than that of idocaine hydrochloride, and its dispersion is similar to that of idocaine hydrochloride. The Chemicalbook has little effect on circulation and breathing. It is non-irritating to tissues and does not produce methemoglobinemia. Common dosages have no effect on cardiovascular function. Large dosages can cause blood pressure to drop and heart rate to slow down. No obvious rapid tolerability.

Bupivacaine Hydrochloride is available in sterile isotonic solutions with and without (as   bitartrate) 1:200,000 for injection via local infiltration, peripheral nerve block, and caudal and lumbar epidural blocks. Solutions of Bupivacaine Hydrochloride may be autoclaved if they do not contain  Solutions are clear and colorless.
Bupivacaine is related chemically and pharmacologically to the aminoacyl local anesthetics. It is a homologue of mepivacaine and is chemically related to idocaine. All three of these anesthetics contain an amide linkage between the aromatic nucleus and the amino, . They differ in this respect from the procaine-type local anesthetics, which have an ester linkage.
Bupivacaine hydrochloride (Marcaine, Sensorcaine) has particularly long action, and some nerve blocks last more than 24 hours; this is often an advantage for postoperative analgesia. Its use for epidural anesthesia in obstetrics has attracted interest because it can relieve the pain of labor at concentrations as low as 0.125% while permitting some motor activity of abdominal muscles to aid in expelling the fetus. The lower concentration minimizes the possibility of cardiac toxicity. Fetal drug concentrations remain low, and drug-induced neurobehavioral changes are not observed in the newborn. Bupivacaine also is approved for spinal anesthesia and is approximately four times more potent and more toxic than mepivacaine and ldocaine. It can be used with or without epinephrin.

Proparacaine hydrochloride and etracain are also indicated to produce local anesthesia prior tosurgical procedures such as cataract extraction and pterygium excision, usually as an adjunct tolocally injected anesthetics. Ophthalmic solutions used for intraocular procedures should be
preservative-free.Preservatives may cause damage to the corneal epithelium if a significant quantity of solutionenters the eye through the incision.
