Risperidone, a benzoisoxazole derivative, is a new generation of antipsychotics. Its active ingredient, risperidone, is a selective monoaminergic antagonist with the unique property of binding with high affinity to 5-hydroxytryptaminergic 5-HT2 receptors and D2 receptors of dopamine. Risperidone also binds to alpha1-adrenergic receptors and, with lower affinity, to H1-histaminergic receptors and alpha2-adrenergic receptors. Risperidone does not bind to cholinergic receptors. Risperidone is a powerful D2 antagonist that improves the positive symptoms of schizophrenia, but it causes less depression of motor function, as well as tonic syncope, than the classical antipsychotics. The balance of 5-hydroxytryptamine and dopamine antagonism in the central system reduces the potential for extrapyramidal side effects and extends its therapeutic effects to the negative and affective symptoms of schizophrenia.
