Beta-Methyl V inyl P hosphate is a pharmaceutical intermediate that can be used to prepare tibipenem pivoxil. Tibipenem pivoxil, chemically known as (+) - (4R, 5S, 6S) - 6 - [(1R) - hydroxyethyl] - 4-methyl-7-oxo-3 [[1 - (2-thiazolin-2-yl) - 3-azacyclobutanyl] sulfur] - 1-azabicyclic [3.2.0] hept-2-en-2-carboxylic acid (2,2-dimethyl-1-oxopropoxy) methyl ester, is a carbapenem antibiotic. After absorption in the intestinal tract, Tibipenem pivoxil is metabolized to Tibipenem, showing antibacterial activity.
Tibipenem has broad-spectrum antibacterial activity against gram-positive bacteria and gram-negative bacteria, including Staphylococcus, Streptococcus, Pneumococcus (including PRSP), and Moraxella catarrhalis (including β- Lactam producing bacteria), Haemophilus influenzae (including ampicillin resistant Haemophilus influenzae) have strong antibacterial activity. In particular, compared to previous oral antibiotics, tibpenem exhibits extremely strong antibacterial activity against pathogenic bacteria such as otitis media, paranasal sinusitis, and pneumonia in children, penicillin resistant pneumococci, and macrolide resistant pneumococci.
The mechanism of action of tibpenem is to inhibit the synthesis of bacterial cell walls, have high affinity for various bacterial penicillin binding proteins (PBPs), and play a bactericidal role.
 Tibipenem has a higher affinity for PBP1A, 2X, or 2B of penicillin-resistant pneumococci than other oral antibiotics β- Lactam antibiotics.
